It is easy to make a referral to New Horizons Counseling Service, Inc!
Please download the printable referral form below, complete the referral form, and fax to 623-939-7365.
A letter will be sent to the client to let them know the referral has been received, and that they may call New Horizons Counseling Service, Inc. to schedule an appointment at any time. Referrals are valid for 90 days from date on the referral.
If you are an individual seeking services without a referral from an agency, please contact New Horizons Counseling Service, Inc. at 623-939-6567 to schedule an appointment.
