What programs do you offer?
New Horizons Counseling Service, Inc. is an adult outpatient behavioral health service agency. The agency offers counseling services for General Mental Health, Substance Abuse, Relapse Prevention, DUI Education and Treatment, Driver’s License Revocation (DMV Registered), Domestic Violence Offenders, Domestic Violence Survivors, Anger Management, Parenting Skills and Education, and Sexual Offender treatment. Psychiatric Nurse Practitioner Services are available for Psychiatric Evaluations and Medication Monitoring Services. How do I sign up for services?
If you are a self referral, you may call New Horizons Counseling Service, Inc. at 623-939-6567 to schedule an intake/screening appointment.
If you are being referred by an agency, please have the agency representative complete a referral. A printable referral form for faxing and the referral process is outlined on this website under “Make a Referral.”
What are your fees?
Please contact the New Horizons Counseling Service, Inc. office for specific information related to our fees. Information is also available on our website under “Methods of Payment and Promotions.”
What types of payment do you accept?
New Horizons Counseling Service Inc. accepts payments in the form of cash or money order only in the exact amount of the fee for service(s). Upon availability, AHCCCS/ Title XIX funding may be accepted. New Horizons Counseling Service, Inc. is also in the process of accepting private insurance plans. Please visit our website in the future to get updates on this status.
What happens if I don’t have the money for my payment?
Treatment services are provided only with payment prior to the service. If you do not have the payment for an intake, you will be offered 2 chances to reschedule and will be assessed a rescheduling fee. If you do not have the payment for a counseling service, you will not be able to attend that service and may receive an unexcused absence.
What if I am on AHCCCS and my AHCCCS stops?
If you are on AHCCCS and your coverage ends, you will be responsible for paying any treatment fees that were not covered by AHCCCS. You will also be responsible to pay any treatment fees for the duration that you are not enrolled in AHCCCS. If you become enrolled in AHCCCS once again, please call 623-939-6567 to notify staff of this change in your insurance coverage status immediately.
What is the difference between a screening and an intake?
An intake assessment occurs when a client has a referral from an agency that indicates a predetermined amount of treatment that is needed. A screening is conducted by a counselor to determine how much and what type of treatment is appropriate. Both appointments are the introductory appointment to the agency, and include completion of required paperwork and treatment assignment.
Do you offer services in languages other than English?
Currently, New Horizons Counseling Service, Inc. offers all services in English. If a client is on AHCCCS/ Title XIX funding, you may be eligible to receive an interpreter as a covered service. Please contact New Horizons Counseling Service, Inc. to inquire about current status of accepting AHCCCS/Title XIX funded clients.
What if I have a specific cultural preference related to my treatment services?
New Horizons Counseling Service, Inc. employs a diverse and culturally competent team of employees. During the intake appointment, a counselor will discuss with you if you have any specific cultural or spiritual preferences that you would like to be incorporated into your treatment, and a plan to incorporate those preferences will be developed during that appointment.
Who do you get referrals from?
New Horizons Counseling Service, Inc. receives referrals from a variety of sources, including adult probation, parole, courts, Maricopa County RHBA, other behavioral health agencies, and self referrals.
What is the level of experience of your counselors?
Counselors and clinicians employed by New Horizons Counseling Service, Inc. have Bachelor’s Degrees, Master’s Degrees, and/or are licensed through the State of Arizona in the Behavioral Health field. They also have extensive training in their specialty areas of counseling. New Horizons also provides internship opportunities for interns in the process of getting their Master’s Degree in counseling or social work.
Can my significant other or family member attend with me?
New Horizons Counseling Service, Inc. welcomes you to bring anyone with you that you would like to have to participate in your treatment to your intake appointment. At that time, your Intake Counselor will ask to sign a Release of Information for any persons you would like to have participate in your treatment. However, non-clients are not able to participate in group counseling sessions due to confidentiality laws.
What information can be shared with others?
New Horizons Counseling Service, Inc. takes confidentiality very seriously, and is compliant with all privacy laws under HIPPA. No information can be shared without the client’s consent and their signature on a Release of Information form. If you are referred by an agency, your signature on the referral form allows New Horizons Counseling Service, Inc. to inform that agency if you have scheduled an appointment with us. A detailed Release of Information form will be completed with the assistance of a counselor at the intake appointment. If you are receiving treatment at another agency and/or want others to participate in your treatment, please ask your Intake Counselor for a Release of Information for those persons.
Can I attend more than one group per week to get my groups done faster?
Treatment is assigned on an individualized basis for a certain number of sessions per week for a set number of weeks. You may only attend groups according to your treatment schedule, and may not attend additional groups per week in order to finish treatment sooner.
Why do different people have different number of treatment sessions assigned?
New Horizons Counseling Service, Inc. prides itself on providing individualized treatment for each client based on their needs. Treatment sessions are assigned based on agency referral, client history, clinical presentation, and client need.
How many absences can I have?
Attendance is necessary at all group sessions for successful completion of the program.
Discharge from the treatment program will occur if you have (three) 3 consecutive
absences or (four) 4 total absences, and you will be required to begin treatment over from the beginning. Treatment is only allowed to start over one (1) time. If you start over in
treatment, payments for each group will also start over.
How can I get an absence excused?
Absences are only excused with appropriate documentation from the referring agency or a doctor for medical or other emergencies, including death of a relative. In order to receive an excused absence, clients must call the agency at 623-939-6567 within 24 hours of the absence to inform staff and provide documentation of the reason for the absence from a medical provider or referring agency to the agency within 3 business days of the absence. The dates of the documentation for the absence must match scheduled treatment dates. Documentation will be reviewed by a supervisor to approve the excused absence.
What if I come late to a group counseling session?
Punctuality is important to the treatment process and it can be disruptive to the treatment process if clients arrive late to groups. If you are late to a group counseling session, you will not be allowed into group and will not be given credit for attending the treatment session.
What happens if I am discharged from treatment due to absences?
If you are discharged from treatment, your referring agency will be notified. You may re-enroll one time in services, and may have to complete a Re-enrollment intake appointment. If you are referred by a court for DUI or Domestic Violence, you may have to get approval from the court and/or go before the judge prior to re-enrollment. Clients referred by City of Phoenix Municipal Court Substance Abuse Screening Services will be referred back to the client’s screener.
What hours are you open?
New Horizons Counseling Service, Inc.’s office hours are from 8am-5pm Monday-Friday. Group treatment is typically held in the evening from 6-8pm, Monday-Thursday. Group treatment and Nurse Practitioner psychiatric evaluations/medication services are available on Saturday’s. Please inquire about Saturday hours.
What holidays are you closed for?
New Horizons Counseling Service, Inc. is closed on New Years Eve, New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, day after Thanksgiving, Christmas Eve, and Christmas Day. Please contact our office regarding holiday closures if a holiday falls on Saturday or Sunday.
